This page contains downloadable pdf's for all sorts of evaluation and information materials. These are divided into 3 categories: for Fellows, for Faculty and for Both.The Fellows should use the "Goals and Objectives" documents to understand what is expected of them on clinical rotations, as well as to evaluate the rotations. Faculty should use the forms to evaluate the fellows. This data is required to assess the teaching mission of the division. These PDF forms also include rotation evaluations.
